Health Insurance
All students taking more than 6 credits are required to submit a Proof of Insurance Form to the Student Affairs Office before class registration can be completed.  Rivendell College students may be insured through the American College Student Association (ACSA).  

Students insured by their parents or who may have some other insurance policy must provide the proper documentation of their insurance to Rivendell College by submitting these forms to the Office of Student Affairs:

1.    Health Insurance Requirement and Acknowledgement Form
2.    Certificate of Immunization for Rivendell College
3.    Rivendell Immunization Form
